Some components of a person centered assessment include \u2013 each identified by the individual and explored in detail:<\/p>\n\n\n\n<ul>\n<li>Personal strengths viewed as an important personal asset<\/li>\n\n\n\n<li>Areas of interest<\/li>\n\n\n\n<li>Areas for improvement in life satisfaction, short \/ long term goals:\n<ul>\n<li>Employment \u2013 what kind<\/li>\n\n\n\n<li>Education \u2013 in what areas<\/li>\n\n\n\n<li>Socialization \u2013 what kind, how frequently, with whom<\/li>\n\n\n\n<li>Leisure \u2013 what kind, how frequently including interests not explored<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Community \/ environmental resources and assets that can be accessed<\/li>\n\n\n\n<li>Challenges \/ barriers:\n<ul>\n<li>Financial, legal, safety, medical, interpersonal relationships<\/li>\n\n\n\n<li>Living situation \u2013 Preferred, realized, including location and with whom<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Mental health \u2014 current status, i.e., what mental health support is in place, coping strategies, challenges, etc.<\/li>\n\n\n\n<li>Desired outcome of services \u2014 where would the person like the process to end up?<\/li>\n<\/ul>\n\n\n\n<p><strong>Examples of questions that can be asked:<\/strong><\/p>\n\n\n\n<ul>\n<li>Personal strengths:\u00a0\n<ul>\n<li>What are you most proud of in your life?\u00a0<\/li>\n\n\n\n<li>What is one thing you would not change about yourself?\u00a0<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Interests and activities:\u00a0\n<ul>\n<li>If you could plan the \u201cperfect day,\u201d what would it look like?\u00a0<\/li>\n\n\n\n<li>What matters to you?\u00a0<\/li>\n\n\n\n<li>What dissatisfies you about your life \u2014 what do you wish you could change?\u00a0<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Living environment:\u00a0\n<ul>\n<li>What are the most important things to you when deciding where to live?\u00a0<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Employment:\u00a0\n<ul>\n<li>What would be your ideal job and why?\u00a0<\/li>\n\n\n\n<li>What skills do you have, for example, artistic, technological expertise, good with animals, strong writer or listener, organizer.\u00a0<\/li>\n<\/ul>\n<\/li>\n\n\n\n<li>Learning and School:\u00a0\n<ul>\n<li>What kinds of things have you liked learning about in the past?<\/li>\n\n\n\n<li>What would you like to learn more about now?<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n","protected":false},"excerpt":{"rendered":"<p>Once the person provides consent to begin, the assessment process can start.
