Recovery Worksheet

The recovery worksheet offers a guide to establishing a recovery plan. The process asks people to identify any symptoms they might experience and how it might impact daily life. As a practitioner, you may need to spend time with people to help them build their awareness / personal observations about these ideas.

 You might help someone complete this worksheet and ask them to…

  1. Consider if you live with symptoms (things related to your illness) and describe the impact of symptoms on your daily life

You might then ask them to…

  1. Identify anything they might do to support health and wellbeing related to the impact of  symptoms on daily life 

Here are some examples to help guide you:

Example #1

Symptom: Feeling anxious

Impact on Daily Life: Avoiding people

Action: With help from a friend, rehearse 1- 2 sentences and talk to one person in the day using those sentences. Slowly increase frequency of contacts, monitor progress, and celebrate success.

Example #2

Symptom: Feeling you are not accomplishing life’s milestones and feeling hopeless

Impact on Daily Life: Feeling uncomfortable talking with your co-workers and ultimately skipping work  

Action: Seek mental health support to identify and engage in strengths. Use self-talk to highlight strengths and try to use those strengths at work. Monitor progress and adjust if needed. Remember to celebrate your success!

Example #3

Symptom: Difficulty concentrating in class

Impact on Daily Life: Stopping school attendance

Action: Work with the school’s disability support to identify strategies that can help you enhance concentration in class. Make sure to monitor progress, adjust as necessary, and celebrate success.

This process should help people: 

  1. Identify challenges
  2. Identify how those challenges impact your daily life 
  3. Identify what helps with these symptoms over time (note strategies may become more or less effective over time and may need adjustment)
